A friend of mine who worked for a very large manufacturer of, among other things, military planes once told the story about a supplier who sent in a drawing of an escape hatch. In case of an emergency the hatch would have ejected using small explosives and opened an opening in the cockpit's roof. Due to miscommunication they designed it the wrong way: the hatch would have been shoot into the cockpit. Kind of reminded me of that situation.
