If a person were to use someone else's insurance card and id for medical treatment, how likely is an investigation and what might happen?

I have yet to find something that I want to do, but can't!!

Scratch your foot?

(...Jesus wept!...what kind of monster have I become?...I should seriously reevaluate my life decisions...oh look, a guy busting his ass on an icy drive! That's funny! I wonder if it's real. Looks a little exaggerated when he throws the shovel. Could be real I guess. I really should make my lunches ahead of time. I spend way to much eating out. Have to go to the store for that though. Cold today...)

What if both parties were complicit? All copays are paid and the policy holder never says anything to the insurance company. Is there a standard procedure for verifying that the insured person is the one who was treated? Not ongoing treatment, but a one time hospital stay.

So do they just verify it's a valid #? I guess that's all they do since I read about the guy using dead peoples ssn. Do you think it would help reduce fraud if some initial investigation were done before hand rather than investigating after the fact?
