As far as I know it was both. They activated the SCRAM to try and fix their experiment without realising the reactor was designed such that the SCRAM would spike temperature, the combination was enough to blow the steam piping which caused the meltdown.

I was in Peru for a month or so a couple of years back, met this old guy and his wife who were covering all of the Americas in their land rover. Cool people, good luck to you!
