Think about it like this. Imagine if a male was abducted by aliens when he was a baby, and they took his brain out of his head and put it into a female body. As he grew up, his brain would still "think" it was male, and would be uncomfortable with the female parts. He'd feel an innate sense that his penis was missing, despite never having one that he could remember.

That's kind of what happens. Genetically speaking, a transperson is whatever sex they were born as, but they were never the gender usually associated with the sex. So, they get surgery to "fix" nature's little screw-up.

The two main problems are that people with reassigned genders typically can't have children as their "real" gender (a male couldn't produce sperm, for instance), and the societal disconnect with the whole thing. The first one is just a question of technology and time, and it will be solved someday.

The second problem is more difficult. A lot of times, people say "Wait, so, if you're a girl that used to be a guy, doesn't that just make you gay?" or something like that. Sexual orientation and gender are two different things, though. Unfortunately, with our culture being what it is, that does happen, and more frequently than it should. Sometimes gay men (or just men who aren't comfortable with themselves) do get confused and convince themselves that they're women, just like sometimes lesbian women (or women who aren't comfortable with themselves) convince themselves that they're men.

However, that does not mean that a transwoman or transman is just a confused homosexual. If someone is legitimately in the wrong body, it's very difficult to live with. Because of the cultural issues surrounding it, though, people are unable to seek counseling or get unbiased diagnoses, which leads to people having miserable existences in the wrong body. If everyone was more accepting and understanding, everything would be better for everybody.
