That's literally the multi-billion dollar question. Over time, addiction develops. More & more opiate is needed to elicit the same response. It's important to know that Opioids don't stop the pain, just block the signals to the brain. NSAIDS like cox-2 inhibitors physically prevent the prostaglandin formation, thus reducing pain/ inflammation. The people that need it, will over time need more of it thus starting the physical and mental addiction cycle. The only way to prevent this progressive addiction is very stringent management (starting with prescribing physicians). This is not necessarily conducive of 'easy' access, thus the continual tug-of-war between ensuring people in pain are treated, and junkies can't just doctor shop.

Honestly, I think eventually they'll have to invent some type of neurological screen, where there is a sci-fi like reading of the patients brain waves (almost like an instant spec scan or something appropriate). "Here Mrs. InVultusSolis, scan your brain here"... [10 seconds], "oh wow, sorry to see you're in so much pain, you'll need to take ____ every 8 hours, come back in 2 weeks".
