My favourite Fez bug on Xbox (after patch 1, not sure if it matters) was during the intro sequence where the game "crashes" and reboots like a DOS prompt, my console actually did freeze and become unresponsive while the screen was glitching out. I was so confused and thought the game was doing some Kojima-style meta thing.

That would be like the bare minimum for a smaller party to begin existing. Really you would have a better result with a mixed party-proportional system though.

All of OP's replies stopped like 2 hours before these questions were asked. It makes sense for them to have no answer.

Issues with interprofessional conduct are a bit separate from the topic at hand, but some good results have been made in Australia and NZ with a "top down" approach from the Royal Australasian College of Surgeons and their Operate With Respect campaign.
