Yep, the Chatsworth train collision. The driver was texting and ran a signal on a single-track alignment without positive train control straight into a freight train on the tracks, killing 25. The texting was significant but there's also the lack of PTC on commuter rail tracks (they all share them with freight trains), which would've stopped the train.
