I think the key phrase in the response you quoted is "people who are addicted to..."
Do you abuse your prescriptions monthly and sit in pain until they are refilled? If so, that's what he means by "access to treatment."
Other addicts DO turn to street drugs to fuel their dependence. People in both situations need help.
